CMO, Montefiore Care Management, is a robust healthcare management company that provides technologically advanced services and interventions to help federal and state healthcare programs and commercial insurers serving Bronx and Westchester members achieve optimal health. For more than 15 years, Montefiore Care Management has had a singular focus on improving clinical quality, enhancing the patient experience and reducing health care costs.

Montefiore Care Management works with a network of more than 3,400 physicians and ancillary providers who provide care to more than 225,000 individuals covered by a variety of private sector and government-sponsored health insurance programs. We utilize proven interventions supported by information technology to be able to assess and manage a patient throughout the care continuum. Our involvement spans hospital care, rehabilitation, outpatient care, professional services, ancillary support, community-based programs, home care, remote patient monitoring and other services that may be required to return a member to optimum health. Our programs are intended to meet the needs of members with complex illnesses, as well as to maintain the physical and mental wellbeing of our healthy members.

Montefiore Care Management also supports its network by providing patient education, provider relations, credentialing services, community health programs, data analysis and reporting, financial services and contact center services.

Montefiore Care Management provides medical care coordination and behavioral health under contractual arrangements with Montefiore's Integrated Provider Association (IPA). For more than 15 years, Montefiore Care Management has managed large numbers of chronically ill patients and has developed a systematic approach called Care Guidance.

As a unit of Montefiore Medical Center, the University Hospital of Albert Einstein College of Medicine, Montefiore Care Management has access to state of the art information technology and ground-breaking and clinical research. In addition, Montefiore Care Management operates the Customer Service Department and manages innovative programs such as Montefiore Medical House Calls, the CMO Care Continuity™ program, the Montefiore MyChart online patient portal, and other health education and promotion programs and services.  Montefiore Care Management is central to Montefiore's position as an integrated health care delivery system and its future as a preeminent accountable care organization (ACO).
